Pain on biting or tapping on the tooth resulting from inflamed periodontal ligament are characteristics of Apical Periodontitis. A radiolucent area around the apex of teeth appears on x-ray, usually diagnosed by a dentist.

ANUG (Acute Necrotizing Ulcerative Gingivitis)

Marginal gingivitis is usually a painless condition. However, an acute form of ANUG is associated with bad breath, bad taste, bleeding gums, and severe periodontal pain. ANUG is aggravated by smoking, stress, poor oral hygiene and is non- contagious. ANUG is treated in multiple visits to the dentist, first debridement of necrotic (dead) gingiva, home care with hydrogen peroxide mouthwash, analgesics, cleaning below gums, both professionally and at home.

Pericornitis

A flap of gums (Operculum) overlying the erupting wisdom tooth in late teen’s causes toothache, redness, swelling, and Trismus (difficulty in opening the mouth). Food debris and bacterial accumulation beneath Operculum as this area is hidden far back in the mouth and is difficult to keep clean.

Cleaning the area under Operculum with antibiotic solution, or warm salt water rinse provides immediate relief. After getting symptomatic relief, the definitive treatment is usually extraction of a wisdom tooth or Operculectomy.

Home care requires maintenance of good oral hygiene to keep the area free of debris to prevent reoccurrence.

People with a habit of clenching and grinding of teeth (Bruxism) during sleep or while awake suffer generalized periodontal pain due to excessive biting forces on teeth.

Over time, there may be tooth wear (attrition) which causes dentin hypersensitivity. Therefore, a routine dental check-up is mandatory.
